The forms below are required prior to your first visit with Dr. Foresman, Dr. Maybee, or Kathi Fennelly, NP. Please take the time to fill out EVERY form and bring them with you to your first appointment.

Getting Established
Are you considering becoming a patient at Middle Path Medicine? Here are a few things you need to know:
- Each patient is required to complete a medical history package (available online to the right or in the office), and to see the Medical Assistant for a Wellness Evaluation before seeing Dr. Foresman for your initial one hour appointment, establishing with Dr. Maybee, getting Intravenous Nutrition Therapy, or establishing with Kathi Fennelly, NP.
- Follow up visits with Dr. Foresman are generally one-half hour long, but you can schedule longer appointments when necessary.
- Middle Path Medicine is a “cash practice”. This means that you will pay for our services when you arrive. We will provide you with the documentation you will need (a “superbill”) to submit to your insurance company for direct reimbursement to you.

Changes You Can Make Today
Did you know there changes you can make before you even see Dr. Foresman? Are you ready for a change but not sure where to start? Dr. Foresman has written two different articles that have all the information you need to start your journey to health and healing. Read The Foundations of Health (or watch the video) for more information on lifestyle habits he recommends as a standard for all patients. He has also developed a Basic Nutritional Protocol, the video for which you can find here.
